members. While the information on this site is about health care issues, it

United States Bone and Joint Initiative does not participate in any political

USBJI has applied to be and expects to be incorporated in the State of Illinois
in the United States of America, which laws and regulations you agree shall
govern the USBJI website regardless of where you reside or where you made your

export and re-import laws and regulations. If any case or controversy arises

Members, Participating Organizations and other Supporters of the Initiative
USBJI
collects certain personal information from its members and supporters to insure
that USBJI membership records are accurate and up-to-date. This information may

- Home or office mailing address,
including street name and name of city or town
- E-mail address (where available)
- Telephone number(s)
- Fax number(s)
- Personal website address (where
available)
- Birth date
- Social Security Administration (SSA)

- Membership status
- USBJI identification number
- Credit card information (if
purchases are made)
- For purchases, a
record of all USBJI books, CD-ROMs, videotapes, audiotapes and patient
education materials ordered
- Information from others who
are not USBJI Members or supporters
USBJI
collects certain personal information from others who are not USBJI members or
supporters who wish to purchase or obtain USBJI materials or to register for an
activity or program. This information includes:
- First and last name
- Home or office mailing address,
including street name and name of city or town
- E-mail address (where available)
- Telephone number(s)
- Credit card

USBJI
collects certain personal information online from others who contact the USBJI
to participate in research surveys or to obtain particular medical information.
This information includes:
- First and last name
- Home mailing address, including
street name and name of city or town
- E-mail address (where available)
- Telephone number(s)
- USBJI does not knowingly collect
information from children under age 13.
- USBJI encrypts sensitive personal
data, such as credit card information, while being transmitted on the
Internet. Credit card information is transmitted securely using SSL
encryption.
- The USBJI website is not set
up to track, collect or distribute personal information about its
visitors. It does recognize the home server of visitors, but not e-mail
addresses. For example, USBJI can determine which Internet service
provider our visitors use, but not the names, addresses or other
information about our visitors that would allow us to identify the particular
visitors to our sites.
How
may this Personal Information Be Used?
- Personal information may be used for
internal USBJI purposes by authorized USBJI staff.
- Personal information will not
be released to third parties except as noted below:
- The USBJI may publish a Membership
Directory.
- Certain limited information (i.e.,
name and address of USBJI Members and supporters) may be provided to other
not-for-profit organizations whose missions are consistent with the mission
of the USBJI, upon the review and approval of the Executive Director.
- Members of the public,
through the USBJI website or through links to supporting organization
sites, may in certain circumstances obtain limited information about the
USBJI Members or supporters, including the Member or supporter's name,
address, and telephone or fax number or email address.
- At the current time, we are
aware of no other circumstances under which USBJI will provide or sell
personal information collected online to third parties.
Changes in Terms and Conditions
USBJI
reserves the right to change or modify these Terms and Conditions at any time,
and you agree to give effect to such changes or modifications upon their being
posted to the USBJI website.
By
participating on the USBJI website, you acknowledge that you are aware of these
Terms and Conditions and this Privacy Policy, that you understand these Terms
and Conditions and Privacy Policy and that you agree to be bound by them. If
